In the past, looking for a spare notebook was probably easier than looking for a computer. I don't know if my hands even work anymore with pen and paper for any task that takes longer than signing a check or credit card receipt. I've long been inspired by an idea I first learned about in The Artist's Way called morning pages. Before delving into its various genres, let’s begin with a basic definition of the essay. If you don’t love what you’re writing, no one else will, either. Every day you write, you'll get beautiful stats that analyze the feelings, themes, and mindset of your words. For example, learn about how often you get distracted, and how fast you write. It's about getting it all out of your head, and is not supposed to be edited or censored in any way. The idea is that if you can get in the habit of writing three pages a day, that it will help clear your mind and get the ideas flowing for the rest of the day. Unlike many of the other exercises in that book, I found that this one actually worked and was really really useful. In this process , light bounces exactly back in the direction from which it came due to a nonlinear optical process. This challenges my understanding of traditional teaching methods where knowledge is the basis for inquiry. Perhaps higher-order learning strategies like inquiry and evaluation can also be the basis for knowledge and comprehension, which are classified as lower-order skills in Bloom’s Taxonomy. For reflective writing, it is important to balance reporting or descriptive writing with critical reflection and analysis. Paraphrasing is reserved for large sections of someone else's writing that you want to convey in your own words. Summarizing puts the main points from someone else's text into your own words. Although basic questions like “what is the thesis? †are important to demonstrate your understanding, you need to interrogate your own assumptions and knowledge to deepen your analysis and focus your assessment of the text. If one were to look into a complex conjugating mirror, it would be black because only the photons which left the pupil would reach the pupil. Critical reflection requires thoughtful and persistent inquiry. Not only the direction of the light is reversed, but the actual wavefronts are reversed as well. The black history in the forms of slavery, racial discrimination, poor wages for workers, denied access to education, and civil rights movement continued to shape the African-American experience even after the abolition of slavery in a late nineteenth century. At the turn of the twentieth century, the Jim Crow segregation laws were enacted in the south which created legally segregated transportation systems, schools, working environments, and lodgings. The African American workers were not allowed to join the northern labor unions and when faced with a threat of a strike, employers employed blacks since they had no movements to advocate for them. The black workers were overworked and under-payed and had to work under dangerous working environments (Herndon, A., 1937). It was until the rise of the civil rights movements in the 1960’s that the racial segregati on was abolished. Although the Blacks were engaged in all major U.S wars, they served as servicemen and were not allowed to participate in combat operations. In 1948, President Harry Truman issued an executive order that allowed the African Americans to be integrated in the US. Army (Engelhardt, B., n.d). This paper will discuss some of the key moments in the twentieth century including the Harlem Renaissance in the 1920s, the workers unions and the trial of the Scottsboro boys in the 1930s, the double ‘V’ campaign and the integration of blacks in the U.S forces in the 1940s, and the non-violent civil resistance in 1960s. The Harlem Renaissance was one of the most significant cultural movement that shaped the African American experience. There was a prodigious movement of the Blacks from the southern villages to the northern cities during the First World War and continued in the 1920s. This migration led to a black cultural renaissance which was known as the Harlem Renaissance and spread through New York City and other cities in the North and West (Locke, A., 1925). The Renaissance became famous as the whites visited the New York’s Harlem to enjoy the exotic adventure in the Jazz clubs, speakeasies, and dancehalls. This was the first major acknowledgment of the African American art, music, and literature. Many people including students, businessmen, peasants, professionals, artists, poets, musicians, workers, preachers, exploiters, criminals and even social outcasts came to Harlem with separate motives. The prejudice and proscription had brought all these dissimilar elements into one area where unity and racial sympathy fused their experience and sentiment (Locke, A., 1925). The mainstream critics and publishers shifted their focus to African American music, arts, literature, and politics (Hughes, 1926). Eloquent writers such as Langston Hughes raised their voice against the discrimination against Negros arguing that the blacks should have the right to choose what they do and do what they choose. The resurgence did not just shape the cultural identity of the African Americans but also led to their amplified political inclusion. The resurgence began as a result of racial segregation but later became an avenue for racial integration between the whites and the blacks (Locke, A., 1925). The injustices in the Southern legal system in the 1930s was exhibited by the unconstitutional conviction of Angelo Herndon and the Scottsboro boys who had been sentenced to death. Jim Crow segregation laws greatly affected the African American workers who were overworked and under-payed (Herndon, A., 1937). Herndon joined the communist party and began to advocate for workers right by uniting the Black and White workers. The African American workers were barred from joining the north unions and any revolt was indicted in court under the insurrection law. The unions were weak due to the division between the whites and blacks. When the white workers go on strike, the employers would call upon the blacks and when the blacks were on strike, the employers would employ more whites (Herndon, A., 1937). Herndon organized a peaceful interracial demonstration in Georgia and was arrested in 1932. He was tried in court before a jury bench who were all white and found guilty of violating the insu rrection law. He was sentenced to a minimum of 18 years in prison. The insurrection law had been enacted in a period of slavery in 1861 and stated that inciting insurrection, revolt, resistance or conspiracy on the part of slaves was punishable by death. This case drew a widespread attention and support from labor unions, religious groups, and civil rights movements. The case was brought to the U.S supreme court where the insurrection law was termed unconstitutional and Herndon was freed in 1937 (Herndon, A., 1937). During this period, the Scottsboro case had sparked protests and echoes that roared all over the world. The nine African American youths from Scottsboro, Alabama had been indicted for having raped two white girls. This case inspired a great hatred of the whites against the blacks. Although the evidence of the rape case was slim, the jury sentenced the nine youths to death. The case was taken to the U.S supreme court twice and overturned the convictions. But the Alabama j ury found them guilty and only four were freed while the rest were sentenced to long-term imprisonment (Herndon, A., 1937). These cases saw the rise of labor unions and civil rights movements. During the World War II, the black servicemen were fought to further the U.S war goals and at the same time fight against racism. This was termed as the â€Å"Double V†strategy as the African Americans were motivated to win the victory overseas and the victory at home (Engelhardt, B., n.d). More than 909,000 African Americans enlisted in the army and 167,000 enlisted in the Navy were organized in separate black units. The black soldiers were given assignments that were spearheaded by racial segregation policy which included restriction from combat responsibilities. The African Americans had to, therefore, stick to non-combat support roles and had to watch as the whites undertake difficult combat operations (Engelhardt, B., n.d). Despite these restrictions and racial prejudice, the African American troops were determined to serve their countrymen and envisioned their own freedom back home. The study also focused on organisms such as metazoans, from which adequate biochemical information exists on the primary enzyme function, and the products excreted to facilitate the presentation of the metabolic maps (Mller, et al., 2012). The review covered protists and metazoans but did not consider metazoans capable of withstanding temporary hypoxia or anoxia in lactate, ethanol, and other fermentations (Mller, et al., 2012). The review centered on metabolic pathways specific to anaerobic eukaryotes and their mitochondria. The authors linked various research findings to create their review and offer an update of the phylogenetic framework. Definitions of various phenomena and processes have also been edited to fit the objectives of the paper. For example, eukaryotes in the paper refer to heterotrophs or growth of that nature because the authors did not consider photosynthetic synthesis of ATP in the paper. It is also suggested that the fire was slowly burning due to lack of oxygen in the dock area but the sudden influx of oxygen made it possible for the fire to migrate to the mail retail showroom (Newman, 2010). Without involvement of nurses, programmers may include items not used by nurses or put charts out of logical order for the nursing process. Handheld Devices The current trend in computerized healthcare management is handheld devices or PDAs. â€Å"The functionality provided by PDAs has expanded exponentially from simple personal organizer to include healthcare databases and applications that check for drug interactions, aid in IV calculations, analyze lab results, provide charge capture information, scheduling functions, prescription refilling and other practice management tasks†(Online Journal of Nursing Informatics, 2001). Using a handheld device, nurses are able to keep current on research, standards of care, and drugs. The nurse will be able to access patient records from the bedside, drug room, or during change of shift report. A PDA will cut down on the spread of germs since each nurse carries her own device instead of sharing a computer keyboard with the entire hospital staff. The handheld device will be able to offer real time updates if there is a change in a patients status, perhaps to â€Å"transmit telemetry waveforms from monitors at the nurses station directly to the bedside nurse†(OJNI, 2012). At change of shift, a nurse can handoff to the next nurse syncing the two handheld devices improving continuity of care and assuring that tasks are not inadvertently left off. Security Standards The Health Insurance Portability and Accountability Act (HIPPA) is federal law in place to protect a patient’s privacy. Any computerized medical records must be in compliance with HIPPA as well as protect the computer records from being breeched in anyway. How to keep medical records safe and away from non-authorized eyes is a major concern for healthcare providers. The technical part of security includes firewalls, protected servers, ncryption, and secure networks. Storage and back up of all information will be integral in protecting the electronic medical records. At Epic Systems, â€Å"’we have all sorts of firewalls and security systems in effect to prevent data breaches,’ Ms. Faulkner says. On laptops used by doctors, files can be viewed but not stored. The same is true for smartphones and tablets. We do not store patient data on them, she says, so it cannot be misused if these devices are stolen†(Freudenheim, M, 2012). Hospitals also need role based access. This will â€Å"limit access to a patient’s record to only those individuals who need the information for treatment. Audit trails can automatically record who viewed the health record and can be used after the fact to identify any unauthorized access, leading to improvements in training or, if warranted, corrective action (HHS. org, 2012). As with paper charting, nurses will need to carefully guard who has access to the patient’s chart and who is allowed to view it. Computer monitors need to go blank when not in use to prevent unauthorized viewing of patient records. Cost The savings to health care systems is not only in time, convenience and increased patient safety; CMRs can also save hospitals money. The Department of Health and Human Services published a white paper outlining cost benefits of transitioning to electronic records. â€Å"The all cost-benefit analyses predicted substantial savings from EHR (and health care information exchange and interoperability) implementation. The quantifiable benefits are projected to outweigh the investment costs. However, the predicted time needed to break even varied from three to as many as 13 years†(AHRQ, 2006). The cost to implement electronic medical records vary tremendously. PracticeFusion is a company that offers an on line free EMR program. This company offers â€Å"a comprehensive practice management software platform includes charting, scheduling, billing, e-Prescribing, lab integrations and secure messaging†(PracticeFusion. com). There are no charges to use the technology. If the user prefers to use PracticeFusion without seeing the ads, they can pay a nominal $100 per month fee. On the other end of the cost spectrum, Epic Systems provides electronic medical records for large hospitals like Cedars-Sinai and Kaiser. â€Å"Kaiser estimates it will eventually spend a total of $4 billion on the software and related osts like those for equipment and training employees,†(Freudenheim, M. , 2012). For large, multi-campus hospital systems, the costs are enormous. There are extra costs associated with CMRs. Training time, upkeep, new hardware, and IT employees all go in to the continuing ? Benefits to Care The benefits of electronic medical records to the patient can be many. Patient safety can be increased, health care providers receive the absolute most current research and evidence based best practices, and timeliness of care can go up as well. Communication between members of the health care team is improved. Medication errors decrease. Time savings from transcription to reading illegible handwriting leads to more time the nurse is able to spend caring for the patient. Even the US government wants to support CRMs. â€Å"The federal government is contributing to the recent growth of Epic and similar firms by offering financial incentives to health providers who switch to electronic records. It sees these records as a way to improve patient care and reduce inappropriate costs like those for duplicated tests,†(Freudenheim, M. , 2012). Recommendation The two providers I looked at are PracticeFusion and Epic Systems. Comparing these two companies is a little like comparing apples to oranges. Epic has been in business developing electronic medical record systems since 1979 and has an excellent reputation in the industry. Epic claims to have the medical records of 127 million patients and is used by 250,000 doctors. In an industry where cross platform communication can be difficult to say the least, it is reassuring to go with a company that has such widespread market share. Epic is relatively easy to learn and use. It is the system I learned while working at Cedars-Sinai in Los Angeles and the system I recommend. PracticeFusion is not in the same league as Epic Systems. It may be a good choice for a small, detached practice to use for internal EMRs ? Justification The cost of using Epic Systems for medical records can be prohibitive. Once the decision to go electronic has been made, Epic works closely with the staff at the hospital to design a user friendly, safe, and effective program. The government is supporting the transition to EMRs. Other hospitals have made the move, private practices are involved. Quality of patient care can be increased and that is really the best reason to make the change.
